Thyroid Eye Disease can cause the eyes to appear more prominent and bulge forward due to swelling and inflammation around the eye sockets. These visible changes may affect a person’s emotional well-being, including their self-esteem and confidence, and can influence social or professional interactions.

What Causes Bulging Eyes in Thyroid Eye Disease?

Bulging eyes, also called proptosis, happen when the tissues behind your eyes swell and push your eyes forward. In Thyroid Eye Disease, the immune system mistakenly attacks the muscles and fat around the eyes, causing inflammation and tissue growth. This process is what gives the eyes their characteristic bulging appearance which can hinder daily activities or affect self-confidence.

Renowned oculoplastic surgeon Dr. Raymond Douglas treats new and existing patients with Thyroid Eye Disease from his offices in Las Vegas, Nevada and Beverly Hills and Long Beach, California. Dr. Douglas also contributed to the development of TEPEZZA, the first FDA-approved treatment for Thyroid Eye Disease.

If you experience bulging eyes due to Thyroid Eye Disease, talk to Dr. Douglas about your treatment options, including Tepezza, as soon as possible.

Tepezza Before and After: Treatment Results

TEPEZZA helps relieve the physical symptoms of Thyroid Eye Disease (TED), such as eye bulging (proptosis), enlarged eye muscles, misalignment (strabismus), pain, redness, blurry vision, and double vision. In many cases, TEPEZZA has been shown to significantly reverse TED, restoring eye function and appearance.

What Does Tepezza Treat?

Tepezza treats the underlying processes that cause the eye symptoms in thyroid eye disease (TED). In TED, biologic treatments often target key molecular drivers such as the thyroid-stimulating hormone receptor (TSHR) and the insulin-like growth factor-1 receptor (IGF-1R). These receptors are present on orbital fibroblasts and immune cells. When triggered by inflammation, the receptors activate the orbital fibroblasts, causing them to release inflammatory cytokines. This process contributes to the buildup of hyaluronan, expansion of orbital fat, and swelling of the extraocular muscles.

Tepezza Can Effectively Reduce or Even Reverse Visible Signs of Thyroid Eye Disease, Including:

  • Bulging eyes

  • Misaligned eyes

  • Extraocular muscle size

  • Pain

  • Redness

  • Double or blurry vision

Could You Be A Candidate For Tepezza?

Tepezza is intended for people with thyroid eye disease. If you’re under the age of 18, talk to Dr. Douglas about using Tepezza, as this treatment may not be recommended for younger patients.

You can use Tepezza treatment if you’ve had previous eye or thyroid surgeries to address your condition, even if you’ve had your thyroid completely removed. Tepezza treatment can still improve your quality of life, providing results comparable to those achieved with corrective surgery.

Schedule a consultation with Dr. Douglas to determine if you could benefit most from Tepezza or corrective surgical treatment like orbital decompression.

Who Should Not Use Tepezza?

Tepezza is not for people who are or expect to become pregnant within six months of the treatment’s 24-week duration or people who are breastfeeding.

TEPEZZA Before and After

TEPEZZA helps relieve the physical symptoms of Thyroid Eye Disease (TED), such as eye bulging (proptosis), enlarged eye muscles, misalignment (strabismus), pain, redness, blurry vision, and double vision. In many cases, TEPEZZA has been shown to significantly improve  TED symptoms, restoring eye function and appearance.

How Does Tepezza Work?

Tepezza (teprotumumab) is a human monoclonal antibody that blocks the IGF-1R signaling pathway. By doing so, it helps reduce inflammation, the buildup of hyaluronan, and excess fat in the eye socket. This leads to a decrease in proptosis (eye bulging) and improvement in symptoms like pain, swelling, and double vision. Together, these benefits can significantly improve quality of life by easing discomfort and reducing changes in appearance.

Clinical trials show that Tepezza improves quality of life by addressing the symptoms of thyroid eye disease.

What to Expect During Tepezza Infusion Treatment

Dr. Douglas offers Tepezza infusions from his treatment centers in Beverly Hills, Long Beach, and Las Vegas. An infusion session takes about an hour. Your infusion is administered intravenously with an IV drip. Most people are able to resume normal activities soon after a Tepezza infusion, and any side effects, such as cramping, headaches, or fatigue, are often mild and temporary.

Other side effects can include hearing-related side effects, increased blood sugar (especially for those with diabetes), digestive issues, and infusion reactions. Dr. Douglas and his team carefully monitor each patient and provide guidance to manage any side effects safely, so you can feel supported throughout the treatment.

How Effective Is Tepezza Treatment?

Clinical studies show that most patients experience a noticeable reduction in eye bulging. In the clinical studies, it moved the eyes back by at least 2 millimeters, which is considered a meaningful improvement. Many patients begin to see results within six weeks, with continued progress throughout the full 8-infusion treatment course.

Your Tepezza treatment plan may include scheduled infusions every three weeks for a 24-week treatment period. Many patients see results within just six weeks of starting Tepezza treatment, after about two infusion sessions. You’ll need all eight treatments in the Tepezza series to prevent your symptoms of thyroid eye disease from reappearing.

Unlike surgery, which physically corrects the eye position after tissue changes occur, Tepezza works best during the active phase of Thyroid Eye Disease by addressing the inflammation that causes bulging. In some cases, this can reduce or delay the need for surgical intervention.
