What are dermal fillers?
Dermal fillers are injectable products that are used to enhance or replenish volume, restore youthful contours, and smooth lines and creases on the skin. Where wrinkle reducers like BOTOX are often appropriate for creases around the eyes and on the forehead, where muscle contractions are common, dermal fillers are often introduced to the lower two-thirds of the face. There are several formulas and different families of dermal filler products available today, including the Restylane and Juvederm family of products. The extensiveness of product choices allows us to achieve optimal aesthetic outcomes for each person. While most dermal fillers are made with the same base ingredient, hyaluronic acid, each product differs in the thickness and consistency of the fluid injected. A thicker, more viscous filler adds structure and is commonly inserted deeper beneath the skin. Thinner, softer formulas are ideal for delicate areas such as the lips.
How long do dermal fillers last?
Dermal fillers break down over time. The results of treatment vary based on various factors. These include the type of filler used, the placement of filler, and the number of injections administered. Once a patient achieves their desired results, touch-up treatments may be needed to maintain youthful-looking skin. Dr. Douglas carefully selects each dermal filler used based on the desired outcome discussed during the initial consultation. Some patients will benefit from more than one type of dermal filler to achieve the extent of rejuvenation they would like. For example, a lighter, softer filler may be used to reduce lip lines or add volume to deflated lips while a second product is administered into the cheeks to enhance contouring in the mid-face. Even when two products are administered in the same visit, their results will diminish differently due to the unique viscosity of each chosen filler.
Generally speaking, fillers with a thicker consistency will last longer. A product such as Voluma may last as long as two years. Most other fillers break down within 9 to 12 months. Am I a candidate for dermal fillers?
Adults of all ages may be good candidates for dermal fillers to reduce the appearance of moderate to severe lines and creases around the nose, mouth, or eyes. The best candidates for fillers are nonsmokers who are in good general health. Smokers can receive treatment, but may not achieve the full extent of beneficial results due to a lack of tissue oxygenation and the squinting and sucking motions made when smoking. Patients with extreme allergies may not be suited to certain types of dermal fillers. Fillers are also not approved for use in pregnant or nursing women.
What are the benefits of dermal fillers?
While we often think of wrinkles and creases as the signs of facial aging, but volume loss is actually more telling. This makes our cheeks flatten and the area around our mouth loosens and develops more lines. Volume loss can mainly be blamed on our declining production of collagen. Every year after we turn 20 our bodies produce one additional percentage point less collagen. Do the math and at 50 that’s a 30 percent decline. Since collagen provides the structural latticework that supports our skin and keeps it firm, this decline creates the volume loss that allows much of our facial aging. Throw in sun damage, personal habits, and environmental factors and our facial skin can really show our age, and then some. Dermal fillers return some of that lost volume. When injected beneath a wrinkle, crease, or area of volume loss the dermal filler instantly fills the space and pushes the skin back upward. This erases the crease and adds volume.
Dermal fillers are an excellent non-invasive option for rejuvenating the face.
What areas can dermal fillers be used?
People confuse dermal fillers such as Restylane® with neuromodulators such as Botox®. But they are completely different in how they erase wrinkles. Neuromodulators work on the upper third of the face, the expressive area of the face. When we make certain expressions these involve muscle contractions. Over time these contractions create wrinkles on thinning surface skin. These wrinkles are known as “dynamic wrinkles.” Neuromodulators block these muscles from contracting, so the wrinkles don’t form. Dermal fillers have no effect on dynamic wrinkles caused by muscle contractions. Dermal fillers only work on the other type of wrinkle, known as “static wrinkles.” Static wrinkles don’t have anything to do with muscle contractions; they are due to sun damage, declining collagen production, environmental factors, and personal habits such as smoking. Static wrinkles form on the mid to lower portions of the face.

How often do you need dermal filler injections?
Different dermal fillers have different lifespans. On average, Restylane fillers last a little less time than Juvéderm®, but again there is variance within the line. You can estimate that Restylane® fillers will provide results from 6 to 9 months. Juvéderm® fillers usually last from 9 to 12 months. Juvéderm® Voluma is the longer-lasting hyaluronic acid filler, restoring cheek volume for up to two years. The difference in metabolism in different patients affects how long a filler lasts before it is absorbed by the body. Generally, areas with more movement break down the filler faster. This is especially true of the lips.
